The mission of the Illinois Department on Aging is to serve and advocate for older Illinoisans and their caregivers by administering quality and culturally appropriate programs that promote partnerships and encourage independence, dignity, and quality of life.
Aging Care Connections’ Aging Well Neighborhood program brings together older adults, businesses, and healthcare and social services to create a “Health Neighborhood” that promotes life-long wellness, supports aging at home, addresses chronic conditions, offers health education, and helps build an age-friendly community in Greater Lyons Township.
AgeOptions and our partner agencies offer programs and services that make it possible for people to continue to live independently in their homes. These include home-delivered meals, assistance in applying for benefits, falls prevention and other programs to help you take charge of your health, programs to address social isolation and assistance with transportation, housekeeping and more
